Application for Assistance
Joey's Journey Foundation is an organization created to foster hope, happiness and to help families during their time of need.
Joey's Journey Foundation will provide financial support to families that have a child going through cancer treatment or treatment for other life-threatening illnesses. We hope families have the opportunity for special moments that take their mind away if even for a brief moment. This could include, but is not limited to, travel expenses, dinner, tickets, goodies or other items to create comfort, provide peace of mind for parents or make a child smile.
→ Joey's Journey Foundation is open to all families with children who have cancer or other life-threatening illness in Indiana or the surrounding states. Indiana will remain at the top of our fulfillment list if/when funds are limited.
→ Families must explain how they plan to use the money and how it aligns with our mission stated above.
→ Eligible patients must have been diagnosed when they were 18 years old or younger. The patient must currently be receiving treatment.
→ Families are eligible to apply multiple times, up to a total maximum of $500.
→ The application must include an original letter from the primary physician or case/social worker on hospital letterhead. The letter should verify the diagnosis and treatment of the patient and be signed by the primary physician.
→ Applications will be reviewed and approved by the Joey's Journey Foundation board on a monthly basis.
→ Families give Joey's Journey Foundation the right to use your child’s name and likeness on our website, Facebook page, and marketing materials.
Families participating in a Joey's Journey Foundation financially supported activity agree to release Joey's Journey Foundation against any and all liability, damages and/or claims of any kind that may be connected with or result from participation, notwithstanding any liability, damage or claim that may arise due to the negligence of Joey's Journey Foundation.
Approved applicants will receive a check payable to the parent/guardian and mailed to the address on the application.
